Free Czech invoice template
A Czech invoice template needs the supplier's name, address and IČO, the customer's details, a unique invoice number, the issue and due dates, the items with prices, and the total. VAT payers add both DIČ numbers, the date of taxable supply and a VAT breakdown. DigiTale Invoice gives you this as an online form that produces a PDF.

A template, top to bottom
The table follows the order in which the parts of an invoice are normally read. The right-hand columns show who needs each part, according to the requirements guide.
| Part | What goes in it | VAT payer | Not a VAT payer |
|---|---|---|---|
| Title and number | “Invoice” or “Tax invoice”, and a unique invoice number | Required (evidence number) | A designation, if you keep accounts |
| Supplier | Name, street, postcode, city, IČO; DIČ if you have one | Required, plus DIČ unless none was assigned | Required; IČO if assigned |
| Customer | Name, address, IČO; DIČ if they have one | Required, plus DIČ unless none was assigned | The parties to the transaction, if you keep accounts |
| Dates | Issue date, due date, and the date of taxable supply (DUZP) | Issue date, and DUZP if it differs | Issue date, and the day of the transaction if it differs, if you keep accounts |
| Items | Description, quantity, unit and unit price; for VAT payers the price without VAT and the VAT rate | Required | The content and amount of the transaction, if you keep accounts |
| Totals | Total due; for VAT payers the tax base, VAT and total for each rate | Required | The amount |
| Payment details | Bank account or IBAN, variable symbol, payment QR code | Optional | Optional |
| Note | Anything else, such as a register entry or a payment instruction | Optional | Register entry if you have one |
The columns summarise the checklist in the guide to Czech invoice requirements, which gives the statute paragraph for every detail.
What the online template produces
- A PDF invoice in English or in Czech, chosen per invoice. Each invoice is printed in one language.
- Two modes: a VAT-payer invoice (“Tax invoice”, with the VAT recap and the date of taxable supply) and an invoice without VAT (“Invoice”, with “Not a VAT payer” under your details).
- Three layouts (Classic, Compact and Statement) and six accent colours.
- Optional payment details with a QR code: QR Platba for Czech accounts, or a SEPA QR code for euro invoices with an IBAN.
- Prices are entered with VAT included; the invoice prints the unit price without VAT and the VAT recap for VAT payers.
- A small “Issued with DigiTale Invoice” line in the PDF footer.
What it is not
- It isn't a downloadable Word or Excel file. You fill in the form and download the finished PDF.
- It doesn't check your details against a register. You are responsible for what you enter.
- It doesn't do reverse-charge invoices (§ 92a), VAT stated in Czech crowns on invoices in another currency, or simplified tax documents (§ 30).
